My name is George Baker - I’m a 19 year old university student from Southampton. I have Muscular Dystrophy, and must use an electric wheelchair at all times.
While that’s partly what has led me to write this report, I am not looking for sympathy, or wishing to make any pull on your heart strings.
To the contrary, my long experience battling, scraping and struggling to get the care I need from our social care system means I know far more about it than I’d like to. It is that knowledge which could help YOU.
The Health and Social Care System Can be a ‘Black Hole’ of Despair
Most of us have (maybe without thinking about it) come into contact with our social care system. If you haven’t yet, you most probably will do in the future!
Have you ever thought about what life is like for people who need social care?
Imagine the following scenario:
Your Grandma, (who always liked to knit you those weird Christmas jumpers you used to complain about but secretly love for their powerful combination of supreme heat retention and woolly goodness!) has contracted Alzheimer’s and is slowly losing her capability to live independently, in fact it has reached the point where someone needs to be there all day because although she can still do a lot by herself, someone needs to make sure she doesn’t leave the gas on and such like.
As much as you would love to, your work and family commitments mean there is no way you can do it yourself and your parents decided to swan off to Spain 5 years ago and have no intention of coming back, so who (considering you are an only child) is going to look after poor Granny Doris?
Surely the government gladly picks up the ‘burden’
Well... That would be nice... And it is sometimes the case - but it ‘aint easy!
The process would go something like so:

Social services will assess Granny’s needs and allocate a funding budget or “care package” to her.

As Granny is still pretty mobile, all she needs is someone to pop in twice a day to get her out of bed and give her breakfast, then give her dinner and put her to bed.
|

The upshot of this is this Social Services would probably pay one of the many registered Care Agencies something like £18 an hour to send someone round at designated times to perform said duties.
|
This should be easy as care agencies are some of the most scrutinized businesses in operation.
Everything they do is governed by rules and regulations to make sure that both the patient and the carer are as safe as possible. And to make sure the agency is legally protected should a patient or employee decide to sue them (in my experience this can get in the way of effective caring, but I understand it’s importance).
Patient opinion of these agencies varies widely. Some praising the reliability and quality of the services they provide and others reporting the complete opposite. In fact, conflicting accounts of the quality of the same agency are far from unheard of.
Trouble Is It’s POT LUCK!
Essentially, it’s “pot luck” as to whether your experience with an agency will be good or bad. That means that although Granny Doris might do just fine with her new care package and love the extra company it brings her. It is just as likely that people won’t turn up and she will miss the toilet or fall and bang her head. Or, she might find that the carers ignore her because she is ‘just another patient’ (which can be surprisingly degrading).
This wide variation of opinion is just as true for elderly care and nursing homes. If someone close to you is going to move to (or is already in) a care home, then what I’ll be telling you in this report is already relevant to you!
Don’t Be Fooled Into Thinking This Has Nothing to Do With You
By this point you may be a little bored and thinking:
“This has no relevance to me whatsoever: I don’t care, I feel for poor Doris (but my Grandma’s name is Marie), it’s boring and depressing, go away!”
...and I wouldn’t blame you.
It does sound very boring and even I get bored of talking about carers and care agencies. BUT... then I remember how important it is!
As citizens of a well developed country there is no reason we should have to think about this at all - and most of us don’t ... Because we assume it will all be sorted for us!
But...The truth is it isn’t sorted for us.
And until a system that works properly and fairly is put in place, we have to think about it if we want to have a good quality life should we become old and frail, ill or disabled. So the point I am trying to put across is:
The Social/Health Care System IS Relevant to YOU!
Nigh on any debilitating illness or condition that results in a loss of mobility, motor function, memory or awareness could well make somebody eligible (or at least deserving of) extra help to live their lives.
This includes things like:

Moreover, old age by itself usually results in a need for extra help as well, so if you plan on living a long time, you will almost definitely need somebody to care for you eventually.
Our care system is an integral (and sadly, to some extent neglected) part of our society. Its effectiveness has a big effect on the way things are, especially with the aging population.
An effective care system would help to negate some of the terrible strain on the NHS, partly by
ensuring vulnerable people are kept in a good state of health, but also by simply providing ways to look after people other than in hospital.
Here’s Another Example
Let’s say that you contract a form of cancer- say lung cancer.
Unfortunately, it gets to a terminal stage and you are looking at options for where you will spend your last days. Obviously, the nicest place to be would be your own home, but that would mean having nurses come in to look after you and the authorities don’t have the money to spare.
The next option is to go into a palliative care home (run by a charity) but there are none close enough for you family to visit regularly, so the last option is to stay in hospital, somewhere I doubt you’d like to spend your last days?
Now... We all know that the NHS does the best with the funding it has, but, something doesn’t quite add up here.
It costs a ridiculous amount of money to keep somebody in hospital, even for a short amount of time. Not to mention that it uses up a valuable bed.
I’m not suggesting that because a person is terminally ill they don’t deserve the bed - quite the opposite. All I’m suggesting is it’s a waste of a bed because it is more expensive for them to stay in hospital than it would be at home - if the correct measures were in place!
So I hear you ask “What can I do to help myself and ‘Granny Doris?’”
Well... Like anything in else you need to:
Learn How the “System” Works and Use It to Your Advantage!
Social care boils down to money and the government don’t like giving out money willy-nilly, you have to earn it (as opposed to deserve it).
You have probably encountered (or at least heard of) needs testing?
This is one situation in which it is used.
In essence, it means (in this context), that people are tested to determine their “care needs”. Based on a set of pre-determined criteria like “can they brush their teeth?” and “do they know the difference between hot and cold?”
Whether the person meets these criteria or not determines how much money they are allocated (obviously if you can’t brush your teeth you will get more cash).
But Here’s What Is SO Important!
Needs testing determines not only WHERE a person’s funding comes from (and yes very often YOU will have to pay at least something towards YOUR care) but HOW it is administered and there are positives and negatives to all the different outcomes.
For example, if it is decided that the NHS should pay then a care agency will probably
be contracted.
But... If Social Services are deemed accountable then a scheme called “Direct Payments” (which I use) will likely be implemented. In this case the service user is paid by Social Services and allowed to recruit carers of their choosing.
With “Direct Payments” the service user becomes an employer and has to run a business.
That entails the paperwork of any other business - which is extremely stressful (but at least you get people you like).
You MUST KNOW What YOU Want to Stand Any Chance of Getting It!!!
It is therefore very important that when someone is being assessed they know what they want the outcome to be because usually a way can be found to achieve the right outcome.
And this isn’t a case of ‘lying’ or ‘fabricating’ the truth - that is a fast path to NO care.
The point is that most of these assessments are down to semantics. Nobody wants to pay so they try whatever tricks they can to get out of it - a MAJOR flaw in the care system - but one YOU must be aware of.
But There’s More
Needs testing isn’t the only form of testing though...there is also means testing.
The difference is simple...
Needs tests determine what help you need and means tests determine whether you can pay for it yourself.
It gets more complicated however because you will only have a means test if your care is deemed to be the responsibility of Social Services rather than the NHS -- because the NHS is always free...at the point of delivery anyway...
And This is Only the Beginning...
Means testing is just one part of the long road to getting an effective care package and there are many other battles (and believe me they ARE battles) along the way.
In fact, I make no bones about how the “getting your care” part of the campaign is only the beginning of the ‘war’!
The entire time you (or one of your friends/family that you are helping) is receiving care you will have to fight to make sure that not only is it of a high standard, but that it doesn’t get taken away!
Just to put it in perspective, as a disabled person who needs a very high level of care (I need someone to care for me 24/7) I have had (and continue) to work extremely hard to acquire and now run my care package. I get funding from several different government funded bodies that allows me to have a carer (officially called a “Personal Assistant”) present at ALL times so that I can live my daily life independently.
When I was about 10 my Mum was becoming ill from caring for me all the time (my Dad died when I was 8) so she approached Social Services to get some help.
After months of ‘arguing’ and testing and meetings they decided to put me on some sort of trial where I would get a carer overnight 2 days a week for about 2 months.
My Mum and I hated it because it was a different person nearly every time and loads of them smelt of booze and fags, in the end it became more hassle than it was worth because Mum had to show them what to do every time a new person came. After the trial ended we were left with nothing again so Mum just gave up and went back to caring for me 24/7.
Over the next few years we managed to get 2 nights a week on the “Direct Payments” scheme, which at least meant Mum could get some sleep, but it still wasn’t enough.
It took at least another 2 years (I was nearly 17) of intensive letters, phone calls and meetings to get to the point where I got funding
for 24/7 care and it was NOT easy.
It was only after my Mum said she couldn’t do it anymore that they did anything about it and even then it still took ages. In fact, my Mum still cares for me when carers are sick or if we can’t recruit enough (like now).
But let me make something VERY clear!
Despite ANYTHING I might say it should never take away from the fact that I am forever indebted to the people that helped me secure my care package.
I cannot begin to explain how amazing it is to have the funding I need and it is a testament to the progression of our society that people in my position can get any help at all (20 years ago I would definitely have been in a care institution), let alone full time support, so to all those involved (who were helpful), I extend
